Community & Home Support (HACC) Southcare's Community & Home Support program is a community based service supporting frail aged adults and younger people with disabilities in the South East metropolitan area namely the residents of the City of South Perth and the Town of Victoria Park. The Community & Home Support Programme assists frail aged and younger people with a disability to remain living as independently as possible in the community. A team of staff and volunteers is able to help in practical ways with the every day activities with which they may need assistance. Southcare has embraced the wellness concept in the delivery of HACC services. By service providers "doing with" rather than "doing for"; residents can continue to be involved and active, maintaining and improving their health and independence and having more confidence to better manage their day-to-day life. The wellness concept has seen dramatic improvements in older people's health and wellbeing, resulting in lower levels of service provision, which inturn allows more people to be supported to stay in their homes. The concept seeks to see ageing as a process of adaptation in which older people are actively involved in adjusting to and managing the balance of gains and losses. It is recognised that ageing isn’t a constant downhill journey but can comprise of ups and downs, not just downs and more downs. It supports a process which enables older people to move from illness to wellness, to regain, in part or full, independence which has been impaired by illness or injury, giving them back much control over their own lives.
