Community & Home Support

Southcare's Community & Home Support program is a community based service supporting frail aged adults and younger people with disabilities in the South East metropolitan area namely the residents of the City of South Perth and the Town of Victoria Park.

The Community & Home Support Program assists frail aged and younger people with a disability to remain living as independently as possible in the community. A team of staff and volunteers are able to help in practical ways with the everyday activities you may need assistance with.

Southcare can assist by providing help with:

  • Domestic Assistance — on a regular basis
  • Gardening Services & Minor Home Maintenance — on a regular basis
  • Information and Support
  • Personal Care
  • Respite for Carers
  • Transport — for shopping, banking, medical and paramedical appointments

How to Access These Services

A member of Southcare's Community & Home Support team will inform you about the services and the eligibility requirements.
The Community & Home Support Program will arrange an in—home assessment for you to identify which of the support services available would best suit your needs.

Southcare's Community & Home Support Program is funded by the Home and Community Care Program or the Department of Veterans Affairs.
