Community Aged Care Packages (CACP)
Community Aged Care Packages (CACP) are individually planned and coordinated packages designed to meet older peopleSouthcare's daily care needs in the community. The intention of CACP is ongoing care and to provide an alternative to residential hostel care for residents of the South East Metro Region. Services provided can include personal care, medication prompts, meal preparation, domestic assistance, and emotional and social support.
Services provided as part of a community care package may include:
Personal Care
Bathing, showering or personal hygiene
Medication prompts
Toileting Dressing and undressing
Mobility Transfer
Fitting of sensory communication aids
Emotional and Social Support
Linking with social activities outside the home to prevent social isolation
Assistance with Daily Living Tasks
Meal Preparation
Laundry Domestic Assistance
Shopping and/or banking
Rehabilitative support or assistance to access these services Support service to maintain personal affairs
Transport to medical appointments
How to access Services
To access a CACP, a person must first be assessed and approved by an Aged Care Assessment Team (ACAT). Then referrals are forwarded to eligible service providers including Southcare.
