Community Aged Care Packages (CACP)
Community Aged Care Packages (CACP) are individually planned and coordinated packages of care designed to meet older people's daily needs in the community. The intention of CACP is low level care to provide an alternative to residential hostel care. Southcare is responsible for providing this service for residents of South Perth.
Personal Care
- Bathing, showering or personal hygiene
- Medication prompts
- Toileting, dressing and undressing
- Mobility transfer
- Fitting of sensory communication aids
Emotional and Social Support
Linking with social activities outside the home to prevent social isolation.
Assistance with Daily Living Tasks
- Meal Preparation
- Laundry and domestic assistance
- Shopping and/or banking
- Support service to maintain personal affairs
- Rehabilitative support or assistance to access these services
- Transport to medical appointments
How to access Services
To access a CACP, a person must first be assessed and approved by an Aged Care Assessment Team (ACAT). Then referrals are forwarded to eligible service providers including Southcare.
