We work in partnership with you and your nominated family and/or friend(s) to design a care plan of services which are focused on YOU; and your needs and goals. We provide an established and diverse range of services that cater to your personal, health, social, environmental and daily living needs; however, we are flexible and creative because we know that each person is unique and has different needs. We are always working with you to discover new ways to make your life more rewarding.

Our long service history means we have strong roots, relationships and partnerships in the community with other service providers and institutions. We are invested in our immediate community and beyond and therefore we are able to provide you with information and links to other complementary services to enhance your wellbeing and connections to the community.

We work with GPs, social workers, allied health providers and other medical specialists where necessary to ensure your care plan maximises your health and wellbeing. As well, we have service partnerships and relationships with specialist organisations to cater for diverse and specific care needs.

Our service coordination is personal and connected and delivered with the intention to expand your well being and opportunities in smaller and larger ways.

How can I access your services?

Southcare is funded by the Commonwealth and State Governments to provide aged care services in your home and through our Social Centre. We provide these services through the Home and Community Care (HACC) Program and Home Care Packages. Access to these services is determined at a free assessment with you by a Regional Assessment Services (RAS) Team or an Aged Care Assessment Team (ACAT) depending on the level of care you need.

Where do you provide your services?

Southcare is funded to provide services within the South-East Metropolitan region. Our service area includes suburbs in the Cities of South Perth, Belmont, Canning, Victoria Park, Armadale and Gosnells.

How much does it cost?

Basic services provided by us through the Home and Care (HACC) Program and subsidised by the Government start from $8 per hour. Higher and more complex need services may be provided through an Aged Care Package funding which costs more depending on the care service agreed with you.

How does it work once I have been assessed to receive subsidised aged care?

A member of our Care Coordination Team will contact you to arrange an appointment to come out and meet you. Your family members or a friend may also be present at this meeting, should you desire. Together. we will create a care plan with you centred on your needs and aspirations. Once this has been agreed, we will both sign off on it and your services will commence. You will receive a copy of the Care Plan for keep at your home.

What about Private Care

Southcare also provides Private Care services which are not subsidised by the Government.
