The Commonwealth Home Support Program (CHSP) is an entry level care program to support older Australians requiring assistance to keep living independently at home and active in their community.

Types of services you can access under the Commonwealth Home Support Program

Based on your assessed and approved needs, you can access the following services:

Personal care

Live and feel your best with a helping hand from our experienced team. We assist with showering, dressing, continence management, medication, and mobility in and out of bed: essential parts of our home care services for seniors that support daily independence.

Domestic assistance

While you’re living your best life, you also want your home to look its best too!

Our caring hands can help you with everyday tasks like vacuuming, mopping, washing, laundry activities, general tidying of surface areas, and collection and unpacking groceries. These aged home care services ensure your space stays safe, tidy, and comfortable.

Social support & community engagement

Stay healthy and supported with our meal preparation and delivery service, allowing you to stay nourished, independent and confident at home with wholesome meals made to suit your needs.

Transport

Getting out and about is an important part of staying independent. Our caring hands can offer transport support so you can travel safely to your usual activities, appointments and social outings.

How does it work?

1  Call My Aged Care on 1800 200 422 or visit www.myagedcare.gov.au to complete the Assessment Eligibility Checker.

My Aged Care is the starting point to access and receive Australian Government subsidised services and caring assistance.  They will let you know what type of package you’re eligible for, or if you need a private package.

2 Complete a Home Care Assessment

If your application is successful, you will be referred for an assessment. This is done in person, at your own home and will be relaxed and person-centred, with you in control.  Assessments are done to national standards with a trained assessor from the Single Assessment System (SAS) team.

3 Receive your package and choose Southcare as your in-home aged care provider.

We will visit you to create a personalised care plan based on your assessed and approved care needs.   We understand everyone’s needs are different and at Southcare we get to know you to gain a better understanding of your needs and work with you to develop the best care plan to suit you.

Who is eligible for Commonwealth Home Support Program?

Eligibility for the CHSP is based on your support needs and age. Your financial situation doesn’t effect your eligibility. To be eligible for the CHSP program you must be aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.

    What are the similarities between the Commonwealth Home Support Program and Support at Home?

    Both the Commonwealth Home Support Program and Support at Home are funded by the Australian Government. This means that if you are assessed by My Aged Care and your approved care needs meet the criteria, you will receive money from the Government to put towards your aged home care services.

    The range of services you can receive under CHSP or Support at Home are similar. It can include clinical support (such as nursing or allied health services like occupational therapy, physiotherapy and more), independence (such as personal care including self care and daily living, medication management, social support, transport, and more) and everyday living (such as home maintenance and repairs, gardening, domestic assistance and meal preparation and delivery). 

    What is the difference between the Commonwealth Home Support Program and Support at Home?

    The Commonwealth Home Support Program is an Australian Government program providing support for older Australians requiring assistance to remain living independently at home and in their own community. The CHSP is designed to provide entry level support to those who may find themselves seeking a little bit of help with some everyday tasks.

    Support at Home is designed to support those who require more complex aged care needs that go beyond what the Commonwealth Home Support Program can provide. This program delivers person-centred care and a co-ordinated approach to the delivery of your in home care services and can be tailored to suit you, in line with your assessed and approved care needs.

    There are eight classifications of Support at Home, and your classification will be assessed by a professional assessor from the Single Assessment Service using a nationally consistent assessment framework to determine which services you are eligible for.  There is one level for CHSP services.

    The amount of Government funding you receive to put towards your home care differs between the CHSP and Support at Home. As the CHSP is designed for those who have lower care needs, the funding is less. As Support at Home is designed for those who have an increased level of care needs, the package funding is higher and you may be required to make a co-contribution towards your care.  This will be determined by Services Australia based on an income and assets assessment.
