Want to know if you’re eligible for home care support services?

To receive in home aged care, you’ll need to be assessed and approved by My Aged Care and the Single Assessment System (SAS) Team.   This team (of medical, nursing, and allied health professionals) will determine the level of support you’re eligible for to help you stay independent at home. Once approved, you can choose from trusted home care providers in Perth, or switch to Southcare for a better fit. 

Our care team can also help with your application to My Aged Care and explain the different funding classifications available.

1 Call My Aged Care on 1800 200 422 or visit www.myagedcare.gov.au to complete the Assessment Eligibility Checker

2 If your application is successful, you will be referred for an assessment. Assessments are done in person, at your own home.  This relaxed, person-centred assessment is done to national standards with a trained assessor from the Single Assessment System (SAS) team.

3 Receive your package and choose Southcare as your in home aged care provider.  We will visit you to create a personalised care plan based on your assessed and approved care needs.
