My Aged Care is the starting point to access and receive Australian Government subsidised services and caring assistance.  They will let you know what type of care you’re eligible for (eg Support at Home or Commonwealth Home Support Program), or if you need a private care.

2 Complete a Home Care Assessment

If your application is successful, you will be referred for an assessment. This is done in person, at your own home (or in hospital) and will be relaxed and person-centred, with you in control.  Assessments are done to national standards with a trained assessor from the Single Assessment System (SAS) team.  You will need your Medicare card, doctor referrals and any questions at the ready. Family members, friends or carers can join you at the assessment.

3 Receive your package and choose Southcare as your in-home aged care provider.

We will visit you to create a personalised care plan based on your assessed and approved care needs.   We understand everyone’s needs are different and at Southcare we get to know you to gain a better understanding of your needs and work with you to develop the best care plan to suit you.

As always, you are in control of your care so have choice and flexibility in the way your support services are delivered.

How much does it cost?

Southcare’s fee structure is simple and transparent, putting you in total control. 

The amount spent depends on how much and how often you use our caring hands and the products you need.

Under Support at Home, the Government will automatically allocate 10% of your funding budget to care management fees. You may also need to make a co-contribution towards your care.  This will be determined by Services Australia based on an income and assets assessment.
