Sharing the care with your friends or family is as easy as 1, 2 ,3

  1. Download and complete the referral from (click the blue button above or below), tell us who you would like to refer and share the completed form with Southcare by emailing your Coordinator at chsp@southcare.org.au or coordinators@southcare.org.au.
  2. The person referred signs a Home Care agreement with Southcare for either Commonwealth Home Support Programme or Home Care Package services*.
  3. Receive your thank you $100 Gift Card in the mail!
