Rather than travelling to specialist clinics or the hospital for your allied health needs, our in-house team will visit you at home at a time that suits you.

At Southcare, our experienced allied health professionals work alongside you to improve your mobility, enhance your quality of life and help you remain independent for longer. By delivering services in the comfort of your own home, we can provide practical, personalised support that makes a real difference to your daily life.

Delivering care in the comfort of your home allows us to assess your surroundings, identify safety hazards, provide recommendations and deliver physical support in a familiar environment where you feel most comfortable.  It means your safety at home is maximised and your physical rehabilitation needs are taken care of in a safe and familiar space.

All services are delivered in line with the strengthened Aged Care Quality Standards, and our occupational therapist and physiotherapist works closely with Southcare’s in-house clinical care and nursing team as well as your family, other Southcare care workers and any other of your chosen healthcare providers.

Southcare’s Occupational Therapist can help you with:

Cognitive support

Our caring hands can offer practical strategies and tools can help you stay on track with everyday tasks and maintain your independence for longer. This can include cognitive support to assist with memory, routines, organisation and daily planning.

Home modifications

After completing an assessment with you in your home, our Occupational Therapist can suggest home modifications such as ramps, grab rails, or bathroom alterations to improve accessibility and safety. These changes can make it easier to move around your home and reduce the risk of injury.  We will also work with your Care Partner to arrange assessment for AT-HM funding.

Energy conservation techniques

Our caring hands can recommend energy conservation techniques to help you manage fatigue while continuing to participate in the activities that matter most. Simple adjustments to how tasks are completed can help you save energy and avoid overexertion.

Managing health conditions

If you have pre-existing health conditions such as arthritis, dementia, stroke, Parkinson’s disease and other age-related conditions, we can work with you to recommend strategies and supports to help you adapt to changes and maintain your quality of life.  Southcare’s Occupational Therapist will also consult with our in-house nursing and clinical care team so your health needs will be treated holistically.

Maintaining independence

Our caring hands can help you maintain independence with everyday activities such as showering, dressing, meal preparation, housekeeping and other daily routines. Support is tailored to help you continue doing as much as possible for yourself with confidence, and in line with your assessed and approved care needs.

Equipment recommendations

The right equipment can make everyday tasks safer, more manageable and less physically demanding.  Some of the equipment support Southcare’s Occupational Therapist might recommend are shower chairs, dressing aids, adaptive kitchen accessories, supportive seating, walkers, mobility aids and other assistive devices. Recommendations are made after an assessment with you and in line with your assessed and approved care needs.

Home safety assessments

The caring hands of Southcare’s Occupational Therapist will work with you to create a home environment that supports your independence and well-being. We do this by visiting you at home to identify potential risks and make daily living easier, safer and more comfortable.

Falls prevention

This includes identifying hazards around your home and recommending practical changes to reduce the risk of slips, trips and falls. This may include improving lighting, removing trip hazards and suggesting safer ways to move around your home.

  • assess your risk of falls and identify any risk factors or hazards
  • provide balance and stability exercises
  • provide strategies to improve your confidence and safety when moving around the home
  • offer advice on footwear and walking aids
  • deliver a vestibular assessment for new or chronic dizziness
Strength, mobility and physical function

Want to improve your strength, movement and flexibility?  Let our caring hands help with:

  • providing individualised exercise programs to improve your strength, range of movement and endurance
  • exercises which are tailored to your current abilities, preferences and goals
  • support attending a local gym, park, swimming pool to exercise
Maintaining independence and everyday function

  • improve your ability to perform everyday activities such as getting in/out of a bed, getting in/out of a car or using stairs
  • work towards meaningful personal goals—whether that’s walking to the shops, gardening, attending social activities or simply moving around the home more confidently
Walking and mobility aid assessment

In the comfort of your home, Southcare’s Physiotherapist will:

  • assess whether you would benefit from a walking aid and discuss the different options available
  • provide education on the correct fitting and use of walkers, frames, sticks and other mobility equipment
  • review your current equipment to determine if it’s still appropriate for you.
Pain management and musculoskeletal conditions

If you are living with pain or any musculoskeletal condition, our caring hands can:

  • assess and treat of common injuries, pains and musculoskeletal conditions in the comfort of your home
  • support you to manage arthritis, back pain, joint pain and stiffness
  • provide education and exercise strategies to help you manage pain and remain active
Post-hospital and post-surgery rehabilitation

If you are returning home from a hospital stay, we can:

  • support you to safely regain your strength and independence after hospitalisation, surgery or illness
  • provide rehabilitation following joint replacement, fractures, falls and other significant health events
  • help you transition safely back to your usual activities at home and in the community
Heart, lung and neurological conditions

Our caring hands can support you with exercises and education to manage:

  • lung conditions including chronic obstructive pulmonary disease (COPD), asthma and respiratory infections to improve your lung function and reduce breathlessness
  • heart conditions including coronary heart disease, heart failure, and recovery after heart attack with a tailored cardiac rehabilitation program
  • neurological conditions including stroke, Parkinsons disease and multiple sclerosis with a tailored neurological rehabilitation program

How it works

Initial consultation
We visit you at home to ask you some questions about your health goals and assess your home environment so that we can best recommend solutions for your health needs and ongoing support.

Personalised care planning
Our caring hands will develop a tailored care plan designed specifically for you, and in line with your assessed and approved care needs.

Home visits
Depending on the support you need, our occupational therapist or physiotherapist will visit you at home to provide ongoing care, monitoring and support.

Ongoing review and support
As your needs change, we adjust your care plan to ensure you continue receiving the right level of support.  If you need home modifications or equipment, we can work with you to arrange assessments as part of your funding under Support at Home.

    I enjoy helping people stay active, independent and confident in their everyday lives, and getting to know each person, their goals and what’s important to them.

    I have experience across hospitals, community rehabilitation and aged care, working with a wide range of conditions. I have a particular interest in mobility, falls prevention, balance, strengthening and helping people remain independent at home.

    I believe physiotherapy should be practical and tailored to each person. I enjoy working with participants, families and carers to find realistic ways to make everyday activities safer and easier.

    Outside of work, I love spending time outdoors, whether that’s surfing, hiking, camping or climbing. I also enjoy travelling and getting away whenever I can. Staying active and being able to enjoy the things you love is something I value personally and bring into my approach to physiotherapy.
