Transfer your care in six easy steps
Choose Southcare
Some providers might not be able to deliver the care you need. At Southcare, we can deliver personal care, social support and domestic assistance under the Commonwealth Home Support Program, in line with your assessed and approved care needs.
Call My Aged Care and transfer your referral code
Call My Aged Care on 1800 200 422, tell them you want to change providers and ask them to re-issue your referral code. Your referral code may start with 1- or 2-, and is followed by 12 numbers. This code allows us to access your records and accept your funding.
Provide written notice to your current provider
Once you have your referral code, you can formally end your existing agreement. Check your notice period and review your Service Agreement — it’s usually 14 or 28 days.
Agree on an “exit date” and confirm this in writing to your current provider.
Say something like “I am transferring my services. My final day of care will be [insert date]. Please prepare my final statement and arrange the transfer of my unspent funds.”
Confirm your start date with us
Your start date should be the very next day after your exit date. Continuity of care is essential and an immediate transfer means there is no gap in your services.
