Understanding Support at Home with Southcare

Support at Home provides Government-funded in-home aged care services so you can stay independent, comfortable and connected in the place you love most — your home.

In addition to the 8 funding classifications, this person-centred care gives you access to services, products, equipment and home modifications to help you remain healthy, active and socially connected in your community.

This includes:

  • A Restorative Care Pathway, to help you maintain and improve your independence through multidisciplinary allied health services.
  • The Assistive Technology and Home Modifications (AT-HM) scheme, for products, equipment and home modifications to meet your assessed needs.
  • An End-of-Life Pathway, to provide dedicated funding to access services and remain at home in your last 3 months of life.

Support at Home is designed to support those who choose to live independently in their own homes and who require more complex in-home aged care needs that go beyond what the Commonwealth Home Support Program (CHSP) can provide.

No matter if you require entry level care or more comprehensive care, our caring hands are here to provide extra support to you so you can keep living independently and avoid residential care at an aged care facility.

What services you can access under Support at Home

Based on your assessed and approved needs, you can access the following services:

Clinical Support

This includes specialised clinical and nursing services to maintain or regain functional and/or cognitive capabilities and will be delivered by our caring hands and allied health professionals.  Types of services will be delivered in line with your assessed and approved care needs, and may include nursing care, occupational therapy and continence care.

Independence Service

This includes support delivered to help you manage activities of daily living and the loss of skills required to live independently. Types of services will be delivered in line with your assessed and approved care needs, and may include help with getting dressed, taking medications, transport or respite care.

Everyday Living Service

This includes support to assist you to keep your home safe, clean and liveable so you can live your best life.  Types of services will be delivered in line with your assessed and approved care needs and may include cleaning, gardening, shopping or meal preparation.

Support at Home Service Classifications

There are 8 funding classifications under Support at Home as follows, and your classification will depend on your assessed and approved care needs. Please note, amounts for each classification are set by the Australian Government and are subject to indexation.

Additional Support at Home Service Pathways

Our caring hands can help you access three additional short-term service classifications based on your assessed and approved care needs, as follows.

Restorative Care Pathway

The Restorative Care Pathway helps you regain independence with daily tasks. Under this pathway, you may:

  • have access to up to 16 weeks of restorative care
  • receive an extra budget of about $6,000 (or up to $12,000 if required) for multidisciplinary allied health services
  • access restorative care separately, or in addition to ongoing Support at Home services
  • have access to assistive technology and home modifications through the Assistive Technology and Home Modifications (AT-HM) scheme if needed.
End of Life Pathway

The End-of-Life Pathway will support you to remain at home in your last three months of life, surrounded by the people you love and the environment you are familiar with.

You can be referred for a high priority Support Plan Review to access the End-of-Life Pathway, even if you’re not already a Support at Home participant.

Under the End-of-Life Pathway, you will:

  • have urgent access to a higher level of funding for in-home aged care services if you have been diagnosed with 3 months or less to live
  • have access to assistive technology through the Assistive Technology and Home Modifications (AT-HM) scheme if needed
  • receive funding of up to $25,000 over 12 weeks, with 16 weeks to use the funds to provide more flexibility.

If you need services beyond 12 weeks, you can request an assessment so you can continue to receive Support at Home services under an ongoing classification. This pathway is to complement other in-home palliative care services you may be eligible for, or receiving.

Assistive Technology and Home Modifications Scheme

The Assistive Technology and Home Modifications (AT-HM) scheme will give you access to products, equipment (such as a wheelchair) and home modifications (such as a shower rail) to meet your assessed needs.

If you are approved for the AT-HM scheme after your aged care assessment, you can access up to $15,000 for home modifications and $15,000 (or more with evidence) for assistive technology. You may be eligible to receive assistive technology funding, home modifications funding, or both.

    (1) Assessments are undertaken by our in-house nursing team.  Service covers a home visit and all related reports and referrals.

    (2) Assessments cover in-home assessment and are a prescription service that covers all reports and recommendations for additional referrals.

    Explaining the Support at Home Co-contributions

    Contributions are an essential part of the Support at Home program, but please note you will only pay contributions for services you receive. Contribution rates will vary by the type of service and your income and assets. An income and asset assessment, like the age pension means test, is used to determine your contribution rate and this is done by Services Australia.  The following co-contribution summary is current as at 1 November 2025.
