The good news is this: you have choice. And switching providers doesn’t have to be overwhelming. With the right steps — and the right support — you can move to a provider who truly understands your needs and helps you make the most of your funding.

Here’s a clear, step-by-step guide to help you navigate the process with confidence.

Step 1: Secure your new provider first

Before you notify your current provider, make sure you have a new one ready to go.

  • Contact your chosen provider and confirm they have availability.
  • Ask for their unique provider name or code (you’ll need this when speaking with My Aged Care).
  • If you are a Support at Home participant, confirm they can deliver your specific care plan within your current budget classification.
  • If you receive services under the Commonwealth Home Support Programme (CHSP), check they have capacity to deliver the exact services you require.

Taking this step first ensures you won’t be left without support.

Step 2: Transfer your referral code

Your Government funding is linked to a referral code. When you are with a provider, that code is active (or “claimed”) by them. To switch, you must transfer it.

  • Call My Aged Care on 1800 200 422.
  • Request to change your aged care provider and ask them to re-issue your referral code.  Your referral code may start with 1- or 2-, and is followed by 12 numbers. This code allows your new provider to access your records and accept your funding.

Step 3: Provide written notice to your current provider

Once you have your referral code, you can formally end your existing agreement.

  • Check your notice period: Review your Service Agreement — it’s usually 14 or 28 days.
  • Agree on a cessation date: Confirm your final date of service (your “exit date”).
  • Send written notice: Email or write a letter stating something like: “I am transferring my services. My final day of care will be [insert date]. Please prepare my final statement and arrange the transfer of my unspent funds.”

Clear communication helps make the transition smooth and professional.

Step 4: Confirm your start date with your new provider

Continuity of care is essential — especially if you rely on support for showering, meals, medication, clinical care, or mobility.

  • Your start date with the new provider should be the very next day after your exit date.
  • Provide your reactivated referral code to the new provider so they can formally accept the referral and activate your funding.

This step ensures there is no gap in services.

Step 5: Sign Your New Service Agreement

Within 28 days of leaving your previous provider, you must sign a new Service Agreement. If you don’t, your funding may be withdrawn.

Before signing:

  • Review your new Care Plan carefully.
  • Check your Individualised Budget to ensure it reflects your needs and priorities.
  • Your previous provider has up to 70 days to calculate your remaining unspent funds and transfer them to your new provider. You don’t need to manage this yourself — it happens between the two organisations.
